Example 2:

Welcome to the (program name). Please present this card at any of the (merchant name) locations to receive rewards and coupons for being (merchant name) loyal customers! Points are calculated with each purchase, so don’t forget to give the clerk your (program name) card! For point balance present this card at any (merchant name) location. We appreciate your business and hope you enjoy the many benefits of (program name). 

Example 3:

This card is the property of (merchant name). It is for the (program name) and is not for credit or charge purposes. The (program name) card is good at the following locations: (list of locations). This card will not be replaced if lost or stolen.

Example 4:

This card is property of (merchant name) Use this card every time you go to any (merchant name) location.  For your protection, notify any (merchant name) location immediately if your card is lost or stolen. This card has no cash value. (merchant name) reserves the right to alter or cancel (program name) without notice. Personal information is kept strictly confidential.

Example 5:

Please present this card at (merchant name) with every purchase. Reward points will be calculated at the point of sale. There is no cash value on this card; it is for data gathering only. For point balance, please present this card at (merchant location). This card is property of issuer. This card has no point value until activated at (merchant name) for (program name).
